About Us

With YOU in Mind Community Care is a nurse-led, CQC-registered home care service supporting adults across Barnsley and surrounding communities.

Founded and led by two experienced Registered Nurses, Justine, a Registered Mental Health Nurse, and Hannah, a Registered Adult Nurse, our service was created around a simple belief: the care we would want for our own families should be the standard of care we provide to yours.

Unlike larger care organisations, we remain personally and actively involved in our service. We know the people we support, their families and our care team, and provide ongoing nurse-led oversight to help ensure consistently high standards of care.

Nurse-led care with a difference

Our combined nursing backgrounds bring extensive experience across both physical and mental health care. This means we understand that good home care is about far more than completing a list of tasks.

It is about knowing the person, noticing the little changes, recognising when something may not be quite right and communicating effectively with families and other health and social care professionals.

We provide personalised home care and support for adults with a wide range of needs, including people living with dementia and Alzheimer’s disease, Parkinson’s disease, mental health needs, physical disabilities, reduced mobility, frailty, long-term health conditions and the effects of stroke. We can also support individuals with more complex or changing care needs and those requiring additional support to remain safely and independently at home.

Our support can include personal care, medication support, companionship, meal preparation, mobility support, assistance with everyday activities and meaningful social interaction, all tailored around the individual.

Continuity matters to us.

We understand how important it is to feel comfortable with the people coming into your home. Wherever possible, we provide a consistent team of carers who get to know the individual — not simply their care plan.

We take time to understand each person’s routines, preferences, life history, wishes and what matters most to them. Care is delivered with dignity, compassion, respect and choice, while encouraging people to retain as much independence as possible.

Reassurance for families

Choosing care for someone you love can feel overwhelming. Families need to know that their loved one is safe, cared for and that someone will speak up if something changes.

Our nursing experience means we understand the importance of recognising deterioration, changes in physical or mental wellbeing, medication concerns, changes in mobility, nutrition and hydration, and other factors that may indicate someone requires further assessment or professional support.

We work collaboratively with individuals, families and other health and social care professionals and communicate concerns appropriately when additional input may be required.

Care built around YOU

Whether someone needs companionship and a little extra help to remain independent, support following illness or hospital admission, regular personal care or more comprehensive support throughout the day, we begin by getting to know the person.

We complete a thorough assessment and develop an individual care plan based around their needs, preferences and desired outcomes. As circumstances change, their care can change with them.

We are incredibly proud to be a local, nurse-led home care provider in Barnsley, offering the professionalism and clinical understanding of experienced nurses alongside the warmth, consistency and personal approach of a smaller care service.
